The mechanism is mechanical, not metabolic: sequential compression reduces capillary ultrafiltration, prevents lymph from flowing backwards into the interstitial space, and supplies the 'muscle pump' effect that normally drives lymphatic return during activity — for patients who cannot generate that pump on their own.
External pressure raises the pressure in the tissue itself, so less fluid escapes the capillaries into the interstitial space to begin with — the swelling is limited at its source, not just moved around.
The chambers inflate in a directional sequence, holding fluid moving toward the lymph nodes instead of refluxing back into the tissue between compressions.

Clinical comparison
The APTA Academy of Oncologic Physical Therapy grades compression as a Grade A intervention at every stage of breast-cancer-related lymphedema. A meta-analysis of 14 randomised trials (n = 1,397) found intermittent pneumatic compression reduced lymphedema incidence after surgery by 64%, and a prospective study of 232 affected limbs recorded a 28% decrease in absolute limb volume alongside quality-of-life gains across seven of eight SF-36 domains at one year.
The trial evidence in post-surgical lymphedema points to sustained pressures at or below 40 mmHg over more than two weeks. Pressure is selected for your condition and tolerance — higher is not better.
In a randomised trial of patients with chronic leg swelling and recurrent cellulitis, compression therapy reduced recurrence from 40% to 15% — the trial was stopped early because the benefit was clear (NEJM, 2020).

The lymphatic system cooperates with the cardiovascular system and participates in the transport of lymph containing proteins, electrolytes, and lymphocytes — which are essential for the body. Lymphocytes control attacks on microorganisms and pathogens that are dangerous to health and life.

Lazy and dormant lymphocytes may miss or not react quickly enough to pathogens, including viruses, appearing in the body. Such a lowered immune response allows them to multiply and spread. Capillaries are the smallest structures of the lymphatic system. Their role is to collect tissue fluid from the interstitial space. Tissue fluid (lymph) contains, among others, water, proteins, large amounts of lymphocytes (immune cells) and enters the bloodstream to exchange substances with blood. Harmful substances which are detected are then collected and exchanged between blood and lymph and carried to the lymph nodes in the lymphatic system, from where they are further transported to be neutralized. This process is based on the difference in pressure between the blood capillaries, where the pressure is high, and the veins, where the pressure is lower. This one-way fluid movement is controlled by the valvulas. The point of contact between the lymphatic system and the circulatory system is the so-called thoracic duct, the role of which is to collect lymph from the upper left part of the body and the entire lower one, transporting it further to the left subclavian vein, and the right lymphatic duct collecting lymph from the upper right part of the body.

Patient safety is our first priority at DakshinRehab in Moosapet. BOA MAX 2 treatment is safe for most patients, but the following conditions require medical clearance or are contraindicated. Always inform your therapist of your complete medical history.
Acute Deep Vein Thrombosis (DVT) — Active blood clots in the legs are an absolute contraindication. Treatment must not proceed until DVT is resolved.
Peripheral Arterial Disease (ABI below 0.5) — Compression is contraindicated where arterial supply is significantly reduced. An ankle-brachial index must be measured before starting compression in at-risk patients, including those with diabetes or chronic leg ulcers. An ABI of 0.5-0.8 requires modified, medically supervised compression rather than standard pressotherapy.
Active Infections or Skin Conditions — Open wounds, cellulitis, or acute skin infections in the treatment area must be resolved before pressotherapy sessions begin.
Severe Heart Conditions — Cardiac failure or severe cardiovascular disease may worsen with increased venous return. Medical clearance from a cardiologist is required.
Acute Thrombophlebitis — Inflammation of veins with clot formation is a contraindication. Treatment may resume after the acute phase resolves with medical clearance.
Uncontrolled Hypertension — Blood pressure must be managed before beginning sequential compression therapy. Patients with controlled hypertension can proceed with medical clearance.
Active Malignant Tumours in Treatment Area — Patients with active cancer in the treatment zone must consult their oncologist before lymphatic drainage therapy. Post-cancer lymphedema management is appropriate with oncology clearance.
